Professional suggestions for insulin pump Special Authority criteria (letter of July 2021 from DHB clinical leads)

Thank you for your request dated 21 April 2023 under the Official Information Act 1982 (OIA) for information relating to correspondence provided to the Diabetes Advisory Committee (formerly the Diabetes Subcommittee). You wrote: 

I would like to be supplied with a copy of the correspondence from April 2021 from DHB clinical leads in relations to the Special Authority Criteria for insulin pump funding reference in section 8.2 of the Diabetes Subcommittee meeting minutes of 24/9/2021 (https://pharmac.govt.nz/assets/2021-09-24-Diabetes-Subcommittee-Record-WEB-VERSION.pdf(external link)). See page 19. 

Please find a copy of the letter referenced in the record of the Diabetes Advisory Committee meeting of September 2021 (as noted above) attached with this response. 

I note that the letter attached is dated June 2021, not April 2021 as recorded in the meeting record. It appears there is an error in the record of the meeting as the letter provided has been retrieved from the packet of information prepared for the committee for its meeting in September 2021 and no other relevant letters or meetings have been identified.
